We are focused on providing personalized and compassionate hospice care services to help patients and families deal with serious illness through end of life. We are a team trained professionals in providing quality care, comfort and support.

Where is hospice care received?

Hospice care is generally provided in-home. We deliver services wherever our patients call home - private homes, apartments, assisted living communities, board and care, and nursing homes.

Diagnoses

Some common diagnoses that are included in hospice care include end stages of the following diseases:

  • ALS

  • Cancer

  • Dementia/Alzheimer's

  • Liver Disease

  • HIV

  • Kidney Disease such as failure

  • Cardiac and Pulmonary Diseases such as CHF/COPD

  • Neurological Disease

  • Stroke

Those who are experiencing multi-symptom decline and stopping aggressive treatment may also be eligible for hospice.​

What is Hospice?

  • Hospice is primarily a concept of care, not a specific place of care.

  • Hospice emphasizes palliative rather than curative treatment and quality rather than quantity of life. The dying are comforted. Professional medical care is given, and sophisticated symptom relief is provided. The patient and family are both included in the care plan, and emotional, spiritual, and practical support is given based on the patient's wishes and family needs. Trained volunteers can offer respite care for family members as well as meaningful support to the patient.
